Overview
In this unit, you will learn how a counsellor can work as part of a client’s larger case management team to provide counselling services in-line with a client’s individualised plan.

SECTION 1: An Introduction to Individualised Plan and Support
This section introduces you to the contexts of individualised support planning and delivery. You will also learn about the rationale and principles underpinning individualised support, and a range of key practice considerations when working with clients who have complex needs.
SECTION 2: Provide Individualised Support SERVICES
In this section, you will learn about individualised plans and how counsellors can use them to work effectively with clients to develop specific counselling plans that support a client’s progress towards their overall goals. This section will also provide you with an overview of the key processes that counsellors follow when providing individualised support services.
SECTION 3: Monitor and Document Support Activities
In this final section, you will learn how counsellors monitor support activities, make adjustments as needed, and complete all relevant documentation. You will also learn more about the critical legal and ethical considerations involved in providing individualised support.
Unit Title
CHCCCS015 – Provide individualised support
Unit Purpose
This unit describes the skills and knowledge required to organise, provide and monitor support services within the limits established by an individualised plan. The individualised plan refers to the support or service provision plan developed for the individual accessing the service and may have many different names in different organisations.

Suggested Hours
Although everyone will work at their own pace, we suggest you allow approximately 75 hours to complete this unit competently, including seminars, tutorials, assignments and any practical activities.

The recommended time frame for this unit is 4 weeks.
Assessment
This unit will be assessed by completing the CHCCCS015 assessments and an associated practical assessment.

- Legal and ethical requirements and how these are applied in an organisation and individual practice, including:
- privacy, confidentiality and disclosure
- duty of care
- dignity of risk
- human rights
- discrimination
- mandatory reporting
- work role boundaries – responsibilities and limitations
- Factors that affect people requiring support
- Practices that support skill maintenance and development
- Indicators of unmet needs and ways of responding
- Risk management considerations and ways to respond to identified risks
Performance Evidence
The candidate must show evidence of the ability to complete tasks outlined in elements and performance criteria of this unit, manage tasks and manage contingencies in the context of the job role. There must be evidence that the candidate has:
